Wright Dismissal Procedure

Dismissal begins at 2:00pm Monday-Friday. On half days, students will be dismissed at 11:00am.

We encourage parents/adults/providers to pick up all students on time each day.

A separate form should be filled out for each student who attends Wright.

Absent Note Form

Please complete this form each time your child is absent. This information will be submitted in your child’s record to ensure absence is coded properly.

Pursuant to School District of Philadelphia’s attendance policy, all absences resulting in a total of three (3) or more consecutive days will require a doctor’s note. For absences that do not total three (3) consecutive days, parents may submit a handwritten note stating the reason for the absence.

  • Notes must be given to the school within three (3) days upon the child’s return to school.
  • If the note is not turned in within the required time frame, the day(s) may not be excused.
  • When a student has been absent, excused with a parent note, totaling eight (8) days (cumulative), all subsequent absences require a doctor’s note.

Student Emergency Dismissal Plan Form

In the event of an early dismissal due to weather or some type of emergency we need your input on how your child will go home or the plans your child should follow. The plan cannot include or rest at “wait at the school, wait in the yard or wait near the school.”  Please complete this form ASAP.

  • If we do not get a response, we will assume and direct your child to walk home immediately.
  • A specific announcement regarding Legacy will be made prior to dismissal.
  • If you have more than one student who attends Wright, you MUST complete the form for each student.

Parent/Guardian Update Contact Info. Form

Please use this form to update contact information related to your child’s Student Information System (SIS) profile.

Information can only get updated by the parent/guardian who enrolled the child and/or is currently legally responsible for the student at the current moment.

Media Release Form

Media Release Policy

At times, student images, voices, videos, work and/or first name may be requested to promote or represent the School District, School District programs and/or School District events in the news media including, but not limited to, television, print, and on the web. Student information may be used by credible news media not affiliated with the School District and on School District websites and official School District social media pages (Facebook®, Twitter®, YouTube®, Instagram®).

Student information is also used on the Internet for general educational purposes and to celebrate student work. Websites, applications and Internet resources may collect or require the use of student images, voices, videos, work and/or first name. When these are available publicly, the School District cannot control who can view or share.

Accordingly, the School District will not post student images, voices, videos, work and/or first name on a School District website, including District social media pages, or any other public website for news media or educational purposes without prior written consent from you as the parent or legal guardian.

Please return this form to your child’s school to indicate if your child’s information may be used on the Internet. This permission will be applicable to any use of student information in the school year in which permission is given and will remain in effect until removed from the website or until consent is withdrawn.

This form is to request permission (in accordance with Policy 815.1) for your child’s image, voice, video, work and/or first name to be published publicly on a School District website, including District social media pages, or any other public website for news media or for general educational purposes.

Permission for Use of Student Image, Voice, Work and/or First Name for News Media, District Communications, and Educational Purposes:

  • I agree to the use of my child’s image, video, voice, work, and/or first name to be used to promote or represent the School District in news media and School District websites and social media pages as outlined above.
  • I agree to the use of my child’s image, video, voice, work, and/or first name to be used for general educational purposes and to celebrate student work on websites, applications and Internet resource.

As parent or legal guardian, you may withdraw your consent at any time by sending a written letter, along with a new form, to the Principal of your child’s school.

Student Ramadan Participation Form

Ramadan

Every year, Muslims fast each day of Ramadan from dawn until dusk, for the duration of the month.  The Islamic Calendar follows a lunar cycle, and while these are the calculated expected dates of Ramadan, they can differ from place to place based on moon-sightings or lack thereof. Some Muslims strictly follow calculations, while others strictly follow moon-sightings, and some follow a combination of both. What Muslims choose to follow can depend on their religious convictions, where they have lived in the world, and what their families practice.

Students are provided a safe space to complete prayers during the school day.  If your student plans to participate (voluntarily) please complete the form below.  Student participants will be monitored and escorted to and from class before and after prayer.
